Enhancing ICU Delirium Care: A Mixed-Methods Evaluation of ABCDEF Bundle Implementation at Sligo University Hospital.

Delirium is a common and serious complication in critically ill patients, contributing to increased morbidity, longer ICU stays, and long-term cognitive decline. The ABCDEF bundle is an evidence-based framework shown to reduce the incidence and impact of delirium when implemented consistently; however, adherence varies across ICUs, and local barriers often limit its effective use. Establishing current practice and staff perspectives is essential before introducing targeted interventions within Sligo University Hospital’s ICU.

This project aims to evaluate current delirium care and assess readiness for ABCDEF bundle implementation through a combination of quantitative and qualitative methods. We hypothesise that delirium occurs at a clinically significant rate, that baseline adherence to the ABCDEF bundle is suboptimal, and that staff experience identifiable barriers and enablers influencing its use. We further hypothesise that a structured training programme will lead to improvements in practice and delirium-related outcomes.

The objectives are to measure baseline delirium prevalence using validated assessment tools, examine current utilisation of each ABCDEF bundle component, and conduct a qualitative study with ICU staff to explore perceptions of barriers and enablers to implementation. Findings will inform a staff training intervention, after which delirium rates will be reassessed three months later.

This project is expected to generate a clear baseline understanding of delirium prevalence and current bundle use, identify factors influencing practice, and support targeted improvements in delirium management. The results will form a foundation for ongoing quality-improvement initiatives within the ICU.
